67 year old white male with history of diabetes, hypertension, and NYHA class III heart failure has chronic kidney disease with a GFR of 20 for greater than 3 months. He is ambulatory without assistive device. He has no history of dementia.

Vignette 2

80 year old black male with history of diabetes and hypertension has chronic kidney disease with a GFR of 20 for greater than 3 months. He is ambulatory without assistive devices. He has no history of dementia.

Vignette 3

67 year old white female with history of diabetes and hypertension has chronic kidney disease with a GFR of 20 for greater than 3 months. She is ambulatory but requires use of a walker. She has no history of dementia.
