  3. 1 Adjusted for age, sex, weight, primary kidney disease, diabetes mellitus, malignancy, cardiovascular disease.
  4. 2 Additional adjusted for weekly Kt/V urea, rGFR, nutritional status, albumin, ferritin, PTH and CRP.
