• Power point that outlines the transplant process from evaluation to post-transplant. The second portion of the presentation focuses on financial preparation, resources for housing and transportation, and caregiver obligations post-transplant.

• Patient Education Handbook that contains information regarding the transplant process, the Kidney and Pancreas Transplant Program selection criteria process, transplantation and hepatitis c/HIV, the role of social services, additional information about nutrition and staying fit as well as information for potential donors and what they can expect throughout the process.

• Discussion with transplant coordinator, nephrologist, surgeon, financial coordinator, and social worker during evaluation.

After evaluation

• Additional discussions with transplant coordinator, nephrologist, surgeon, financial coordinator, and social worker after evaluation.
