Schematic representation of treatment regimen. Cyclophosphamide was administered orally at 2.5 mg/kg daily for 1 week, then 1.5 mg/kg for daily for 7 weeks. The cyclophosphamide dose was adjusted for renal function as delineated in the methods. Prednisone was initiated at 60 mg daily, tapered to 15 mg daily by 4 weeks, and then slowly tapered to complete a 6 month course as described.
